What is foot arthritis?

Osteoarthritis of the foot is more common in older adults or those with a history of joint injuries.
- Rheumatoid Arthritis (RA): An autoimmune condition, RA causes the immune system to mistakenly attack healthy joint tissues. In the foot, RA often affects both sides equally, and the resulting inflammation can lead to significant deformities if not properly managed.
- Post-Traumatic Arthritis: Injuries to the foot, such as sprains, fractures, or dislocations, can damage the joint cartilage. This damage might not become apparent until years later, leading to chronic pain and stiffness, known as post-traumatic arthritis.
- Gout: While it is a type of arthritis, gout is caused by a buildup of uric acid crystals in the joints, typically affecting the big toe. Gout flare-ups can be extremely painful and are often triggered by dietary factors like high consumption of red meat, alcohol, and seafood.
How is foot arthritis diagnosed?
If you suspect you have foot arthritis, getting an accurate diagnosis is essential for managing the condition and slowing its progression. A podiatrist will typically begin with a detailed medical history and physical examination to assess your symptoms and look for common signs of arthritis. During the physical exam, they will check for joint swelling, tenderness, stiffness, and any visible deformities or irregularities. They may also assess how well you can move the affected joints and observe your gait to see how arthritis is affecting your mobility.
After the physical exam, additional diagnostic tools are usually necessary to confirm the diagnosis and determine the extent of the joint damage. These may include:
Imaging studies
- X-rays: One of the most common diagnostic tools for arthritis, X-rays provide clear images of the bones and joints in your foot. They can show joint space narrowing, bone spurs (osteophytes), and signs of cartilage deterioration. X-rays are particularly useful for diagnosing osteoarthritis, as they can highlight the changes in bone structure that occur as cartilage wears away.
- Magnetic Resonance Imaging (MRI): An MRI offers a more detailed view of the soft tissues in the foot, including cartilage, tendons, and ligaments. This can be especially helpful in detecting early-stage arthritis that may not be visible on an X-ray. MRI scans are also useful for diagnosing rheumatoid arthritis or post-traumatic arthritis, as they provide detailed images of inflammation, joint damage, or soft tissue injuries.
Blood tests
For rheumatoid arthritis, blood tests such as the rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) tests are used to detect the presence of specific antibodies linked to the disease. Blood tests can also check for markers of inflammation, such as C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR), which are elevated in many inflammatory forms of arthritis. In the case of gout, a blood test can measure uric acid levels, as high concentrations of uric acid can lead to the formation of painful crystals in the joints, particularly in the big toe.
Joint aspiration
In some cases, especially when gout is suspected, a podiatrist may perform joint aspiration. This procedure involves using a needle to withdraw fluid from the affected joint for analysis. Examining this fluid under a microscope can help identify the presence of uric acid crystals (for gout) or inflammatory markers (for rheumatoid arthritis).
Treatment options for foot arthritis

- Disease-Modifying antirheumatic drugs (DMARDs): Prescribed for rheumatoid arthritis to slow down the immune system’s attack on joints and prevent further damage.
Physical therapy
